Important Notice -- 2007 EEO-4 SURVEY
Forms 164 and 164a (Hawaiian version) were noticed to have missing or incorrect return addresses. The forms have been updated. Please be sure to direct all correspondence, including completed paper forms, to the EEO-4 Reporting Center, PO Box 1898, Chicago, IL 60690-1898.
